| Medical Device Therapy | Medical Device Therapy An intrathecal pump is an implantable drug delivery system that allows the patient to receive very precisely calibrated, small doses of a pain-relieving drug delivered directly via catheter to the intrathecal region. Programable, implantable pumps for treatment of variety of conditions such as diabetes, cancer, pain and spasticity disorders were in clinical study in the late 80s. An implantable pump for delivery of intraspinal morphine was first made commercially available in the United States in 1991. Implantable pumps have gained acceptance. An implantable pump can reduce the dosage of pain medication by as much as 300:1. Although implantable intrathecal pumps are associated with higher upfront costs than drugs, over the long term they are cost effective when compared to oral therapies. The devices are safe, effective, and require minimal maintenance or programming. An implantable pump is contained entirely within the body. This gives the patient maximum mobility and freedom to pursue such everyday activities as walking, exercising, or swimming. Not all people with chronic pain are suitable candidates for intrathecal pumps. Most chronic pain is best treated with a combination therapy approach. Patients who derive adequate and reliable relief from drugs and other remedies are not suitable candidates. As with most device therapy, the development of the implantable pump is a roadmap of innovation. Some intrathecal pumps are so-called “constant flow” systems which deliver a steady flow of medication. Today, some pumps offer programmability, that is they can be adjusted, even after implantation, to change how the medication is delivered. Further refinements include changes in the footprint, size and weight of the devices (to enhance patient comfort) and devices that allow for easier refilling, longer service life, and more versatility in terms of what types of drugs can be used.
